Understanding OET

What is OET — and why choose it over IELTS?

OET (Occupational English Test) is the only English language test designed exclusively for healthcare professionals. Unlike IELTS, which tests general English, OET tests you in the language you actually use every day — medical consultations, clinical reports, patient handovers.

🏥
Choose OET if you are a Healthcare Professional

OET — Built for Healthcare

All four sub-tests use medical scenarios. Writing and Speaking are profession-specific — a nurse writes a referral letter, a pharmacist explains medication instructions. Your medical vocabulary is an asset, not irrelevant.

  • Accepted by NMC (UK nurses), GMC (UK doctors), AHPRA (Australia), NMBI (Ireland), NCNZ (New Zealand)
  • Writing and Speaking customised per profession
  • Medical terminology you already know is an advantage
  • Faster to prepare for — typically 4 to 6 weeks
  • More relevant to daily clinical work abroad
📝
Choose IELTS for general study or migration

IELTS — General English Test

IELTS tests general English across diverse topics — architecture, tourism, environment, history. For healthcare professionals, these topics are unfamiliar and can be more challenging than medical scenarios despite equivalent English ability.

  • Required for university admissions and general PR/migration
  • Writing Task 1: data description (graphs, charts, maps)
  • Non-medical scenarios can disadvantage healthcare workers
  • Accepted globally for almost all purposes
  • BSL also offers IELTS coaching if this is your requirement
💡
Not sure which test you need? NMC, AHPRA, GMC — which one accepts OET?
Most healthcare regulatory bodies in the UK, Australia, Ireland, and New Zealand accept OET as an alternative to IELTS. Book a free BSL demo class and our certified counsellors will confirm which test your specific regulatory body requires — at no charge.

🎧

Practise with authentic consultation recordings
Focus: medical terminology in spoken context
📖
Reading

Part A: Scan 4 short texts quickly — 15 minutes
Part B: 6 short texts, match headings/sentences
Part C: 2 long texts with MCQ — careful reading
Focus: medical texts, research extracts, clinical guidelines
✍️
Writing
1 Task · 45 Minutes · PROFESSION-SPECIFIC

The most profession-specific sub-test. You read a case notes extract and write a letter relevant to your profession — a referral letter, discharge summary, or handover communication. Assessed on: Purpose, Content, Conciseness, Layout, Language.

Nurses: Referral letters and handover communications
Doctors: Referral, discharge, specialist letters
Pharmacists: Medication advice, prescription letters
All: BSL provides profession-specific letter templates + feedback
⭐ Profession-specific — practise your exact scenarios
🗣️
Speaking
2 Role-Plays · 20 Minutes · PROFESSION-SPECIFIC

Nurses: Patient admission, discharge planning, education
Doctors: History-taking, test explanations, referrals
All: Breaking bad news, managing anxious patients
BSL: Live 1-on-1 speaking mocks with certified examiner
⭐ Profession-specific — real clinical role-play scenarios
OET Grading System

OET grades A to E — what each means for your career

OET uses a letter grade system (A to E) with a corresponding numerical scale (500 to 200). Grade B (350+) is the standard requirement for most healthcare regulatory bodies worldwide.

A
Expert
Scale: 450–500
Near-native clinical command
B
Very Good
Scale: 350–440
Standard pass for NMC, AHPRA, GMC
C
Good
Scale: 300–340
Below pass — most bodies need B
D
Fair
Scale: 250–290
Significant improvement needed
E
Limited
Scale: 200–240
Foundation preparation required
Most regulatory bodies require Grade B in all four sub-tests. Getting a B in three sections and a C in one means a fail — you must re-sit that sub-test. BSL's OET preparation tracks your progress in each sub-test independently and focuses extra attention on any section where you fall below Grade B.

Healthcare Professionals Who Got There

Real OET scores. Real healthcare careers abroad.

Over a million students have trusted BSL since 1971. Here are some healthcare professionals who achieved Grade B with BSL and went on to register and work in their chosen country.

A Complete Guide for Indian Healthcare Professionals

How to achieve Grade B in OET: strategies for every sub-test

OET Grade B (350 out of 500 per sub-test) is the gold standard required by UK NMC, UK GMC, AHPRA, Ireland NMBI, and most English-speaking healthcare regulatory bodies worldwide. Here are sub-test-by-sub-test strategies drawn from BSL’s profession-specific OET coaching for Indian healthcare professionals.

The two sub-tests where Indian healthcare professionals most commonly fall below Grade B are Writing and Speaking. Reading and Listening are generally stronger for Indian candidates with clinical experience. BSL’s live sessions focus heavily on the Writing letter format and Speaking clinical role-play, which are the most technique-dependent tasks.

Writing Sub-test — Most Critical
Write the correct letter type with full professional structure

The OET Writing task requires writing a referral letter, discharge letter, or transfer-of-care letter specific to your profession. Grade B Writing needs: correct letter header format (date, addressee, subject line), all case notes information addressed accurately, professional medical language appropriate to the recipient (a specialist letter uses different language from a letter to a GP), no information invented or embellished from the case notes, and a word count of 180-200 words. The most common reason Indian healthcare professionals fail Grade B in Writing is not addressing all bullet points in the case notes. BSL instructors mark every Writing task against the actual OET Writing band descriptors.

Speaking Sub-test — Clinical Role-play
Manage the consultation, not just the language

The OET Speaking test consists of two 5-minute role-plays with an interlocutor playing a patient or carer. Grade B Speaking requires: initiating the consultation appropriately, gathering information with open and closed questions, managing the patient’s concerns and emotional responses, explaining clearly in plain language (no medical jargon with patients), and closing the consultation professionally. Indian healthcare professionals often score below Grade B because they use medical terminology with patients and struggle with responding to emotional cues. BSL Speaking role-play simulations are tailored to your specific profession and realistic clinical scenarios.

Reading Sub-test
Part B multiple matching: the hardest task for Indian professionals

OET Reading has three parts: Part A (45-minute summary task with four short texts), Part B (3 short workplace texts, multiple-choice), and Part C (2 longer healthcare articles). Indian healthcare professionals generally score well in Parts A and C because they involve healthcare-specific content. Part B, with its workplace English (notices, policies, guidelines), can be challenging because the vocabulary and register differ from clinical English. BSL Reading coaching covers all three parts with timed practice using materials from actual healthcare workplace contexts.

Listening Sub-test
Part C: healthcare professional discussions, not patient consultations

OET Listening has three parts: Part A (patient consultation, form-filling), Part B (six short healthcare professional discussions, multiple-choice), and Part C (two extended healthcare professional discussions, note completion and multiple-choice). Parts A and B are generally manageable for Indian healthcare professionals. Part C discussions use advanced healthcare professional-level English vocabulary. The second Part C audio is a healthcare professional interview with academic or specialist vocabulary. BSL Listening practice specifically targets Part C with accent variety and advanced healthcare vocabulary drills.

Mock Tests — Essential
At least 5 full profession-specific mock tests before test day

There is no substitute for full OET mock tests taken under exam conditions with profession-specific materials. BSL provides 15+ full OET mock tests across all 12 healthcare professions. Each Writing mock test includes written band-descriptor feedback identifying exactly which criteria you met and which you missed. Each Speaking mock includes recorded feedback from your BSL instructor using the OET Speaking criteria: linguistic, clinical communication, and overall communication management.

Time Management
Writing: 45 minutes for one letter — use every minute

The OET Writing sub-test gives 45 minutes to write one letter of 180-200 words. This seems generous but most Indian healthcare professionals who write slowly under pressure do not finish or produce a rushed second half that loses marks. Recommended time allocation: 5 minutes to read and annotate the case notes, 5 minutes to plan the letter structure, 30 minutes to write the letter, 5 minutes to review for grammar and spelling errors. BSL timed Writing practice builds this discipline from the first session.
